What happens after a match is made?

  1. Once the provider has been selected, click “match” next to the identified provider.

2. An automated email will be sent to the selected provider that says, “A client has selected you on Duett for this care request <request link>. You should now connect with the care manager to begin coordinating services.”

3. The Case Manager can then make contact with the provider to provide the information needed for the referral process. At this time, Duett does not share any of the client demographics with the provider. The CM must send the requested information to the provider via email or another format.
