How do I edit client demographic information?

NOTE: If a care request has already been submitted and demographic information needs updated, the case manager MUST delete the care request. Demographic information cannot be edited in a care request that has already been posted.

FOR EXAMPLE:

  1. Care Request 369 for Abraham Lincoln has been submitted and appears on the dashboard. The CM realizes the client’s address needs updated after the care request has been submitted.

2. When the CM clicks “edit” the “Patient Info” section is grayed out and unable to be edited.

NOTE: Other sections of the care request can be edited which can be found here.

3. The case manager must DELETE this care request. Click Delete.

4. Select “Client information entered incorrectly.”

5. A new care request must be entered inorder to edit the client’s demographic information. Since this client has an existing profile, the CM must “search” for the profile when entering a new care request. Select the correct profile.

6. After the CM verifies the correct profile is pulled up. The CM will then edit the needed information and continue through with the care request. Once the care request is submitted, then the demographic information will save.
